Abstract
dc:descriptionReforms the national health systems take place in the majority of economically developed countries in the world thus becoming a different in intensity and scope but continuous ongoing process over the last decade. Pressure for reform in health care comes two-way: the impact of factors outside the health sector, affecting the determinants of national health policy and impacts within the health care system related to organizational, structural and financial challenges. Governments of different countries worldwide and particularly in Bulgaria are looking for options to invest in solutions increasing efficiency and reducing costs, providing better health care to more people. Integrated care is an approach aimed at optimizing processes, improving usability of resources and increasing the effectiveness of healthcare provided. Development of conceptual models for change in the healthcare system is dictated by the high incidence and prevalence of chronic diseases in the twenty-first century. The development of integrated care is a challenge not only for Bulgaria but also for other countries: it’s a challenge from the perspective of patients and health professionals on one hand, and in the management of resources to provide health and social services, on the other. With the close down of the dispensaries in 2010 which happened as part of the Bulgarian health care system reform, chronically ill patients are mostly GPs’ concern. Despite the specific needs of this group of patients, they are treated by the health system as all other users of health services, which creates prerequisites for a number of gaps and duplication of certain activities in their service. The need to introduce and develop models of integrated care is determined by the strong fragmentation that exists between medical and social services, health care professionals, continuous care providers and social services. Creation and implementation of a conceptual model for integrated care would solve the problem of fragmented services and the lack of coordination and balance between ambulatory and hospital care.
