We asked: which parameters are essential for an international PPF registry? A two-round Delphi consensus with purposive and snowball sampling recruited 30 orthopaedic surgeons (final response rate THA n=25, TKA n=20) from 13 countries. Anonymized, mobile-responsive questionnaires synthesized evidence around Vancouver, Rorabeck, Su and Felix classifications, stability assessment, diagnostics, treatment and outcomes. Consensus (≥75% agree/strongly agree) was reached for all Round-1 and Round-2 items, defining a minimum dataset spanning patient factors, index arthroplasty history, mechanism/timing, bone quality, infection work-up, imaging/classification, implant/stability descriptors, management pathways, and outcomes/complications. This registry-ready specification enables harmonized data capture across institutions and supports quality improvement and comparative effectiveness research.
