Design, Implementation, and Evaluation of a Self-Measured Blood Pressure Monitoring Program in a Free Clinic Setting

BACKGROUND: Hypertension (HTN) is a major public health challenge affecting nearly half of U.S. adults. Only one-fourth of those with HTN have their condition controlled, with lower control rates observed among low-income racial and ethnic minority groups. Self-measured blood pressure (SMBP) monitoring has been shown to promote HTN self-management and improve blood pressure (BP) control, but few studies have assessed SMBP in medically underserved populations. OBJECTIVE: To assess the impact of a program combining SMBP monitoring with education and counseling on BP control and knowledge and self-efficacy regarding disease management in a free clinic setting serving a primarily low-income Latinx population. METHODS: Adults ≥18 years old with HTN or elevated BP at a free clinic were enrolled in a 3-month program. Participants received a BP monitor to measure and log BP daily and received education and counseling on HTN self-management. Pre- and post-intervention surveys were administered to assess knowledge about HTN (score range 1-2) and self-efficacy for managing chronic disease (SEMCD; score range 1-10). BP logs were scanned at 1 month. Paired t-tests were used to compare mean knowledge score, SEMCD score, systolic BP (SBP) and diastolic BP (DBP) pre- and post-intervention. RESULTS: Participants (n = 38) had mean knowledge score of 1.32 (SD = 0.33) pre-intervention and 1.92 (SD = 0.12) post-intervention (p < 0.05) and mean SEMCD score of 7.80 (SD = 1.54) pre-intervention and 8.96 (SD = 1.25) post-intervention (p < 0.05). Participants who completed 1 month SMBP (n = 30) had mean SBP of 136 mmHg (SD = 11 mmHg) pre-intervention and 131 mmHg (SD = 12 mmHg) post-intervention (p < 0.05) and mean DBP of 86 mmHg (SD = 7 mmHg) pre-intervention and 82 mmHg (SD = 7 mmHg) post-intervention (p < 0.05). CONCLUSION: A program combining SMBP with education and counseling is associated with significant improvements in BP control and knowledge and self-efficacy regarding disease management in a free clinic setting.
