Cross-Training Home Health Clinicians in Advanced Illness Management

<p>This project focused on the cross training of home health clinicians in Advanced Illness Management (AIM), in order to improve patient care. This project allows patients to receive more timely and patient centered care, by reducing delays caused by having separate clinicians for home health and AIM, and eliminating the need for multiple assessments to identify patients’ care needs. The clinical leadership theme of this CNL project is <em>Care Environment Management</em>. The CNL role is Advocate. The facility that this project was done is a home health agency in Northern California, made up of sixteen nurses, thirteen Physical Therapists, three Occupational Therapists, and one Speech Therapist, Social Worker, and home health aide. To identify the need for this project, a SWOT, and root cause analysis were done. A review of HCAPS, patient outcome results, and nurse surveys were looked at to further identify the need for cross training in AIM. An evaluation of the readiness for change showed that overall the clinicians were interested to learn more about AIM, but had hesitations about being knowledgeable about palliative care. Overall, 63% of clinicians thought the cross training would help improve patient care, and 53% felt better about the cross training after the initial training was given. </p>
