Reducing the Length of Stay for Patients with Stable/Nonoperative Pelvic Fractures by Working with a Multidisciplinary Team

<p>This project is focused on improving the care environment by acting as a clinical outcomes manager. The global aim of this project is to decrease length of stay for patients ≥65 years old with stable/nonoperative pelvic fractures by 0.5 days in a community hospital, verified by the American College of Surgeons as a Level III Trauma Center. This will be accomplished by improving communication between Case Managers, Social Workers, the Trauma Program Manager, and Trauma Surgeons. Trauma Services will utilize and implement the use of an electronic application that is well established in other departments of the hospital to improve communication between the multidisciplinary team. A daily rounding tool will also be used to help prompt questions about the plan of care. Implementation of the mobile application and rounding tool began on May 26, 2016. A fishbone diagram was created to help problem solve delays in discharge. The project ended on August 1<sup>st</sup> 2016. In conclusion, the length of stay increased by approximately 0.5 days. A Root Cause Analysis (RCA) was performed and barriers were identified.</p>
