Patient Centered Medication Education Monitoring: Validating the Efficacy of Patient Medication Education in Promoting Healthy Patient Outcomes

<p>The goal of this project is to assess the efficacy of the patient medication education program, a quality performance improvement initiative being implemented by the hospital to address new and changing healthcare requirements. We are working on this now due to changes in the healthcare field, specifically the federal government’s announcement in January 2015 regarding goals and timelines for shifting Medicare reimbursements from the Fee-For-Service Model to a quality of service based model, with the goal of providing quality care, conserving healthcare dollars and promoting better healthcare outcomes. The quality of service based model and medicare reimbursement rate changes in 2018 and 2016 respectively both recognize and reward hospitals that implement and meet quality requirements. We aim to improve patient outcomes by educating the patient on their medications, presented in a cause and effects analysis, and showing the effects of the patient education efforts. In order to do this, an assessment of multiple factors that could contribute to the effectiveness of the patient medication education program were considered. The project was carried out over a period of 3 months. The majority of the patients surveyed did not show a high degree of knowledge about their medications, frequency, doses and refill information. The findings of the project also showed that patients who were educated about their medications were more favorable to taking their medications as prescribed. The project created a survey which served a tool to gauge the effectivity of the patient medication education program, while at the same time help nurses modify their education intervention depending on patient feedback.</p>
