This project aims to improve medication reconciliation upon admission and discharge to 90 percent within four months in Partial Hospitalization Program (PHP). PHP is an intensive day treatment program for adults who need crisis intervention or short-term stabilization. It also helps clients make the transition from inpatient care to home. Before and after implementation, we conducted a workflow assessment, staff surveys, and chart audits. Before implementation, medication reconciliation was completed 60% of the time upon admission and 25% of the time at discharge. We have clarified the roles and responsibilities. I created a PowerPoint presentation with many screenshots to show the physicians and nurses step-by-step how to reconcile medications in EPIC. I also created a tip sheet for them. All the physicians and nurses have attended the medication reconciliation training except for one psychiatrist who is on vacation. We have addressed some EHR design issues, so it requires less clicks and less time to complete the medication reconciliation process. Therefore, we are making good progress, although the post implementation data collection has not yet finished. The expected outcomes are a reduction in medication errors and hospital re-admissions and an increase in staff and physician satisfaction.
