Nursing staff will report a 100% confidence rate and intent to provide needed CHF education, and a long-term CHF readmission rate goal of 22% will be achieved by December 31<sup>st</sup>, 2016.</p> <p><strong>Methods: </strong>Five proposed activities were to: 1. Increase nursing knowledge of patient resources; 2. Advertize free community CHF class on May 5<sup>th</sup>; 3. Create ‘Heart Failure Zones’ patient education standardization; 4. Observe staff nurse consistency with the teach-back method<strong>; </strong>and 5.
