The specific aim is to improve and standardize diabetes education for the inpatient with diabetes by June 2015. The microsystem is a medical surgical unit for patients 18 years and older who present with diabetes as either a primary diagnosis or secondary diagnosis. Fifty percent of the diabetes patients on this unit are ages 65-89 years old, twenty nine percent are ages 50-64. The unit has 32 beds and is filled to capacity ninety five percent of the time. The fishbone diagram revealed our microsystem lacked diabetes certified staff, standardized diabetes educational materials and identified knowledge deficits among the bedside nurses. The original aim was determined and measured outcomes were performed using the PSDA cycle and PDSA cycle. This allowed for the standardization of tasks that are fundamental for continuous improvements and are embedded into daily practice. Developing actions plans were achieved using a multidisciplinary approach through multiple collaboration sessions. Action plans included evidence based in-patient and out-patient education classes that enabled the facility to follow the patient throughout the continuum of care seamlessly. In-patient, one on one teaching was implemented to identify urgent patient needs related to self-management of diabetes education. Recommendations are to refine curriculum's to interrelate the needs of patients by using PDSA cycles and current evidence based practice guidelines.
