Neutrophil-to-lymphocyte ratio and survival outcomes in testicular cancer: A systematic review and meta-analysis

Abstract

Antecedentes y objetivos: La proporción de neutrófilos/linfocitos se ha utilizado como factor pronóstico en varios tipos de cáncer urológico, como el cáncer de riñón, el cáncer de vejiga, el carcinoma urotelial del tracto superior y el cáncer de próstata. La predicción de los resultados de supervivencia general y supervivencia libre de progresión para pacientes con cáncer testicular se evaluó con el marcador NLR. Métodos: Se realizó una revisión sistemática y un metaanálisis. A priori, se incluyeron todos los estudios observacionales que tenían el valor NLR como exposición y supervivencia libre de progresión y supervivencia general como resultados; finalmente, solo se recogieron 6 estudios tipo cohorte. El riesgo de sesgo se evaluó mediante la Newcastle-Ottawa-Scale (NOS). Resultados: Los estudios incluidos tuvieron un total de 1144 participantes, correspondientes a 6 estudios de cohortes. Conclusión: Existe una mayor probabilidad de progresión de la enfermedad y mortalidad cuando los pacientes tienen NLR elevado pero esto resulta no ser estadísticamente significativo. Debido a la poca evidencia que lo respalda, el marcador NLR en el cáncer de testículo no se recomienda en la práctica clínica.Background & aims: The neutrophil / lymphocyte ratio has been used as a prognostic factor in various urological cancers such as kidney cancer, bladder cancer, upper tract urothelial carcinoma, and prostate cancer. Prediction of overall survival and progression-free survival outcomes for testicular cancer patients was evaluated with the NLR marker. Methods: A systematic review and meta-analysis were carried out. A priori, all observational studies that had the NLR value as exposure and Progression-free Survival and Overall Survival as outcomes were included; finally, only 6 cohort-type studies were collected. Risk of bias was assessed using the Newcastle-Ottawa-Scale (NOS). Results: The included studies had a total of 1144 participants, corresponding to 6 cohort studies. Conclusion: There is a greater probability of disease progression and mortality when patients have elevated NLR but this turns out not to be statistically significant. Due to the little supporting evidence, NLR in testicular cancer is not recommended in clinical practice.
