Comparing a Two-Dimensional and Three-Dimensional Assessment of Unilateral Alveolar Cleft Repairs: A Pilot Project

This study compared the results of a two-dimensional (2D) and three-dimensional (3D) analysis of unilateral alveolar cleft repairs with regards to the percentage of bone fill and an associated bone fill score. Ten sets of pre-operative and post-operative computed tomography (CT) scans were used to determine the defect volume and reverse-segmentation was used to determine the graft volume. Panoramic reconstructions were produced from the CT scans to determine the alveolar defect height and bone fill height. The Bergland analysis and a similar 3D grading index were used to assign bone fill scores. Regarding 2D and 3D bone fill percentages, the Linear regression revealed no significant association, while the Pearson correlation (0.17, p = 0.63) suggested a weak but non-significant correlation. For the 2D and 3D bone fill scores, the Fisher’s exact test revealed no significant association (p > 0.05), while Cohen’s kappa showed a low but nonsignificant agreement (Kappa statistic = 0.13, p > 0.05). The results of this Pilot study suggest that there is no clear relationship between the 2D and 3D bone fill percentages and bone fill scores for any given CT scan set. Regarding bone fill percentages, 2D measurements overestimated as often as they underestimated in relation to 3D measurements. 2D bone fill scores overestimated success more often than they underestimated in relation to the 3D bone fill scores. A lack of agreement between 2D and 3D measurements suggests that clinicians should consider using 3D technology for more accurate assessment of outcomes.
