Our study addresses this gap by collecting information from adolescent female transplant patients who are on mycophenolate mofetil and their guardians within our free-standing children’s hospital. The study surveyed these patients and their caregivers in order to elucidate adolescent girl’s contraceptive knowledge and attitudes. METHODS A cross-sectional descriptive research design was utilized. The study included menstruating females within our free-standing tertiary care children’s hospital who were prescribed mycophenolate mofetil after solid organ transplant and their guardians. The sample consisted of all female adolescent patients who have completed a solid organ transplant and are on mycophenolate mofetil from April 2016 through May 2017. A confidential survey approved by the institutional review board was distributed to patients and guardians at their appointments. Consent and assent was obtained prior to the survey by participants. Descriptive statistical analysis was performed and data analyzed for outliers, and missing data. Data were collected and entered into REDCap and analyses were performed with Stata. RESULTS Twenty-two patients were eligible for the study. Twenty-one patients were approached. Nineteen patients and 17 guardians completed the survey, of which 15 were patient-guardian couplets. The majority of guardians were mothers (94%) and one father completed the survey. The average age of the patient was 16.2years (+/- 3y, 11-22y). The average age at time of transplant was 12.2 years (+/-4.3y, range 1-19y). Race was predominantly white (71.4%), with black (14.3%), and Hispanic (9.5%) as the other groups represented. The type of transplants includes Renal (57.1%), Heart (23.8%) and Liver (4.8%). There were 6 patients (33.3%) who had a history of sexual activity with the average number of partners in the last year of 1.2 (SD+/- 0.4). The number of medications patients took ranged from 4-11 medications a day; average of 8.2 (+/- 2.6) by patient report. Menstrual concerns included dysmenorrhea (10% vs 24%), irregular bleeding (5% vs 24%), and heavy bleeding (10% vs 14%) pre- and post- transplant respectively. More patients were recommended to use contraception after transplant. Use of contraception also increased after transplant. Patients and guardians reported contraceptive counseling prior to and after transplant approximately half of the time. Knowledge of contraception was evaluated using 23 questions published by Guttmacher in their 2009 Survey of Young Adults; using this approach approximately 60% of participants had a failing grade in knowledge and no participants scored a B or better. CONCLUSIONS Adolescent transplant patients have multiple reproductive needs including contraception, dysmenorrhea, and irregular bleeding.
