Transfer of Knowledge From a Physician Residency Training Program

While physicians seek to keep patients well, much of their training and practice focuses on providing sick care to those who are ill or injured. In 1994, the Texas Department of Health began an initiative to promote clinical preventive services using the “Put Prevention Into Practice” (PPIP) model. Included in the initiative was funding to implement PPIP in physician training at family practice residency sites. This study examines if physicians trained in this clinical prevention model are more likely to report that they utilize the skills, knowledge and abilities learned during residency in their current practices than physicians not trained in PPIP. This information can be used to determine if funding family practice residency programs on the pretense of that this training will impact their future practice patterns is a wise investment of taxpayer dollars. A written survey was mailed to physicians who graduated in 1996 from the three funded residency programs and three non-funded residency programs (N=66). The physicians at the funded programs received PPIP education during their last two years of residency. With 39 physicians responding, the results showed very few significant differences in the clinical prevention practices between physicians exposed to PPIP during their residency training and those physicians who did not see PPIP during residency. Perhaps a reexamination of either the clinical prevention education curriculum taught to the residents or the impetus for funding residency programs to insure an appropriate return on the investment of state and federal monies.
