Une réadaptation cardiaque hybride intégrant des séances en centre et à domicile : comparaison des effets à un programme traditionnel et exploration de l’expérience au programme hybride

dc:description.abstractIntroduction: Cardiac rehabilitation (CR) improves the quality of life and functional capacity of people with cardiovascular disease. However, the benefits tend to dissipate after six to twelve months. A CR program that integrates center-based and home-based sessions (a hybrid program) could help maintain these benefits. Objectives: 1) Compare the effectiveness of a hybrid CR model for people with cardiovascular disease to a traditional CR model (center-based only) and assess maintenance of gains six months after the two types of CR. 2) To explore patients’ perceptions and healthcare professionals’ perceptions of patients’ experiences with the hybrid CR. Methods: Objective 1. A pragmatic randomized clinical trial was conducted in nine centers. The traditional program consisted of twelve weeks of center-based exercise sessions. The hybrid program consisted of six weeks of center-based exercise sessions, followed by a gradual increase in home-based sessions for an additional six weeks. Physical activity levels, functional capacity, quality of life, and symptoms of anxiety and depression were assessed at baseline, discharge, and six months after the program. Objective 2. A descriptive qualitative study was conducted. Semi-structured interviews were conducted with sixteen patients and twelve healthcare professionals. The data were analyzed thematically and inductively. Results: Objective 1. Of the 295 participants recruited (146 hybrid and 149 traditional), 224 completed the discharge assessment (113 hybrid and 111 traditional), and 169 were followed up at six months (91 hybrid and 78 traditional). Both types of CR improved all dependent variables at discharge (p < 0.05) and maintained these improvements six months post-rehabilitation. Objective 2. The analysis revealed four main themes: the convenience of the hybrid CR program, bolstering of confidence and mental health, education on eating habits, and physical activity related behaviors. Conclusion: Since both types of CR are equally effective, the hybrid program's strength lies in its greater convenience and flexibility for patients.
