Le développement et l’évaluation préliminaire d’une formation intradisciplinaire sur la gestion optimale des symptômes comportementaux et psychologiques de la démence : une étude pilote

dc:description.abstractBackground: Behavioural and psychological symptoms of dementia (BPSD) are highly prevalent symptoms in institutionalized residents with neurocognitive disorders. Although the existence of many clinical guidelines aiming the management of these symptoms, caregiving staff mention a lack of knowledge and skills in the management of BPSDs. Staff training in this domain therefore appears necessary. Several BPSD staff training interventions exist internationally. However, there is a strong need for the creation of training interventions adapted to the Québec context and for high-quality research to strengthen existing evidence, ensure feasibility and reproducibility. Objectives: This study aims to: 1) develop an intradisciplinary training on BPSD management, 2) evaluate the feasibility and acceptability of this training and 3) evaluate the feasibility and acceptability of selected outcome measures in prevision of a randomized controlled trial (RCT). Methods: A sequential exploratory mixed methods pilot study in two successive phases was conducted in two settings in Quebec, Canada. The first setting was a typical long-term care (LTC) unit and the second, a hospital unit regrouping older people in loss of autonomy with alternate levels of care. Phase 1, the development of the training, was composed of three steps, including focus groups (FG) with staff from 1) both study settings and 2) a panel of provincial experts to identify the training content and modalities to prioritize, as well as 3) a series of cognitive interviews of the preliminary version of the training to allow its improvement. Phase 2 was the administration of the training and the evaluation of its feasibility and acceptability. Following the administration of the training, FGs were conducted with staff from both units having received the training, to evaluate the feasibility and acceptability of the training, and the chosen methodology and measures. Valid and reliable questionnaires were also administered before and after the training to preliminarily evaluate its effects on staff distress and stress. Results and conclusion: This study resulted in the creation of a staff training in the form of five interactive online capsules, administered virtually. Overall, the training and the selected questionnaires were evaluated qualitatively and quantitatively as feasible and acceptable. Preliminary results suggest that staff distress after the training could decrease, but this requires further investigation. The next step will be to evaluate the efficacy of this training intervention formally in a RCT, on both staff and resident outcomes, as well as the sustainability of this type of intervention.
