Recently the principles of Risk Managment have be en introduced in the healthcare system and are rapidly evolving towards a real change of culture: first priority now becomes patients ' security. In the province of Que bec, several legal processes, including the obligation ta reveal accidents ta the patients and the obligation to declare incidents and accidents to the authorities, have been put in place and should facilitate the transition from the well anchored blame culture and silence culture. Complementary measures, such as apology acts, integration of dispute prevention and cofjlict resolution principles, and increased protection of confidential information, should be explored. Lastly, the global process would also be enhanced if incidents and accidents occurring in private practices were to be included.
