Although respiratory symptoms were noted as early as 1817, they are still underestimated today. This thesis explores the symptoms, recognition, impact, and treatment of respiratory dysfunction in Parkinson’s patients. A qualitative study identified four profiles of respiratory dysfunction and their consequences: • Profile 1: Loss of automatic breathing. • Profile 2: Episodes of breathlessness or rapid, shallow breathing triggered by physical exertion, fatigue, postural deformities, stress, or anxiety. • Profile 3: Stress and anxiety triggered episodes of breathlessness. • Profile 4: Decreased cough strength and more frequent coughing. A survey of 939 patients showed that 44.3% experienced respiratory symptoms, categorized into dyspnea (breathing difficulty) associated with female sex, greater rigidity and anxiety and dystussia (coughing difficulty) associated with swallowing and speech complaints. Early recognition of these symptoms may prevent complications like pneumonia. Dopaminergic medication can help, but may also cause biphasic episodes of breathlessness and reduced respiratory function. Respiratory training, such as EMST and IMST, proved effective. EMST and air stacking improves swallowing and coughing; IMST enhances voice (phonatory) function.
