In the current care structure, this often leads to fragmented care and poorer health outcomes. Interprofessional collaboration, in which different healthcare professionals work intensively together, can improve this. This thesis investigated a new interprofessional care unit started in 2020 for complex elderly patients in hospital. The results show that patients on this ward stayed on average two days shorter in hospital, required 69% fewer medical consultations and, on the contrary, received 23% more paramedical care, such as physiotherapy. After discharge, they visited the emergency room 61% less often and had 51% fewer outpatient appointments. This approach proved cost-effective, led to more efficient use of resources and improved collaboration between healthcare professionals. This thesis shows that interprofessional collaboration improves all aspects of care for complex elderly patients.
