Communication among different information systems is a common requirement. However, most often, there are problems with interoperability, caused by various forms of heterogeneity. Main focus of this work is the support of interoperability between various service providers in health care. The main question is, how to meaningfully combine relevant standards and how to mediate between them. A mediator-based architecture is presented that employs domain-specific standards and enables interoperability in shared care. The structures of relevant standards are uniformly specified in Unified Modeling Language (UML) and then appropriately relates them to each other. The resulting models and meta models are meant as a guideline for structural analysis of candidate standards. The developed architecture and meta models are evaluated in a prototype implementation of the epidemiologic cancer registry of Lower Saxony.
