Arterial Calcification and the Clinical Implications on Stent Function

For patients with peripheral arterial disease (PAD), endovascular stenting can be a promising therapy that alleviates plaque obstruction and restores blood flow. However, stent fracture is a serious problem and it can lead to vascular complications, including restenosis. Stent fracture can be affected by type of stent and local mechanical forces that are influenced by arterial wall composition. In this study, an automated computer model platform was devised with meshes inferred from patient specific intravascular ultrasound (IVUS) data, which defined the histological disease condition of the arteries. Combined with stent modeling, this platform is targeted at the development of virtual evaluation of stent performance in a specific arterial setting. Arterial calcification, observed clinically with mild to severe levels, was investigated to determine the impact on stent performance. The analysis demonstrated that arterial calcification does significantly affect stent function, resulting in a 60% increase in logarithmic strains within the stent.
