A DNP project : implementation of a monitoring agreement for patients receiving antipsychotic treatment

This quality improvement project evaluated the implementation of a standardized antipsychotic monitoring and informed consent form within an outpatient psychiatric practice. The aim was to improve documentation of informed consent and adherence to guideline-based metabolic monitoring among psychiatric mental health nurse practitioners (PMHNPs). Guided by the Iowa Model of Evidence-Based Practice and implemented through Plan-Do-Study-Act (PDSA) cycles, the intervention involved integration of a structured consent form into clinical workflows, supported by provider education and ongoing monitoring. A pre-post design was used, including chart audits and provider surveys. Thirty charts were reviewed (15 pre-intervention, 15 post-intervention). Baseline documentation of monitoring agreements was 0%, increasing to 53% post-intervention. Provider survey results indicated positive perceptions of usability, improved confidence in patient education, and enhanced documentation consistency, though workflow barriers limited full adoption. Findings suggest that implementation of a standardized monitoring consent form is a feasible and effective strategy to improve documentation practices, though integration challenges within the electronic health record impacted uptake. Continued refinement and system-level supports are recommended to enhance sustainability and adherence.
