Acute uncomplicated cystitis in an urgent care setting : to culture or not culture--a DNP project

Problem: About half of the 150 million people worldwide who are seen and treated for urinary tract infections annually are women (Mlugu et al., 2023). Majority of urinary tract infections are uncomplicated (Alijanov et al., 2020). In an urban urgent care setting without a standard practice guideline for the treatment of acute uncomplicated cystitis, the providers prescribe a medication that is not first line recommendation. Purpose: The purpose of this Doctor of Nursing Practice project is to define acute uncomplicated cystitis and develop standard practice guidelines for treatment of acute uncomplicated cystitis. The guidelines will include forms of testing, whether a urine culture is warranted, and whether antibiotics should be stopped for cultures with no growth. Methods: A retrospective chart review was performed at one location for an urgent care type clinic in Battle Creek, Michigan to determine how acute uncomplicated cystitis was treated and to determine the need for provider education. A maximum of 350 charts for patients that met inclusion criteria over an 11-month period would be obtained. The information was utilized to develop a standard practice guideline for the treatment of acute uncomplicated cystitis. Results: The study found the primary antibiotic prescribed was the recommended second line treatment 54.32% of the time. The duration and frequency were also above the recommended amount. The first line recommended treatment was prescribed 29.86% of the time and was prescribed for a duration longer than recommended by 38.55% of the time. The rate of urine culture positivity was 68.35% for patients who met the diagnosis of acute uncomplicated cystitis based on literature. Conclusion: This retrospective data collection demonstrated the poor treatment of acute uncomplicated cystitis in an urban urgent care setting. The evidence-based treatment guideline is to be rolled out amongst the remainder of the acute care setting for the institution.
