Intraoperative antibiotic redosing in high blood loss cases for adults

Intraoperative antibiotic redosing following a high blood loss event is necessary to prevent surgical site infection (SSI). Surgical Care Improvement Project (SCIP) measures were created to instill standards into facilities to prevent costly complications associated with SSI. The purpose of this quality improvement initiative aims to improve antibiotic redosing in high blood loss cases at a level one trauma center located in mid-Michigan. While the current literature regarding intraoperative antibiotic redosing is limited, the available evidence points towards the redosing of antibiotics when blood loss exceeds 1,500 mL. An algorithmic protocol was created to facilitate adherence among anesthesia providers. Monthly compliance data were collected and assessed via electronic medical record (EMR). Root causes analysis was conducted and proper interventions were initiated following several setbacks. While the overall percentage of pre-implementation and intra-implementation compliance rate did not significantly change, it should be noted there are less peaks and valleys in the intra-implementation phase and a consistent rise in antibiotics redosing adherence.
