Abstract
dc:descriptionThe chronic instability of the knee due to an anterior cruciate ligament defficiency was reconstructed in 32 cases, between 1985-1989, using the tendons of semitendinosus and gracilis muscles. The Semitendinosus tendon was used in 25 patients. In 5 patients, apart from the semitendinosus tendon, the gracilis was used as an additional reinforcement, while in 2 cases the operation was combined by an extrarticular method. Follow up examination in 30 cases is reported. Patients were evaluated using objectives and subjectives criteria at an average of the 35 months after surgery. With the operation described we were able to achieve functional stability in 93% of the cases. In 9 of the cases patients arthroscopy was carried out 10 to 20 months postoperatively. Despite the difficulties caused by the fat pad flap, arthroscopic examination and biopsy was possible in all 9 patients. In 7 of them tension of the graft was very satisfactory while 2 grafts were loose. In all knees the tension increased during extension. In seven knees there was macroscopic evidence of revascularization of the graft. Histological examination revealed the presence of dense connective tissue. In 2 cases, there was difinite development of vascularized neoplastic connective tissue on the surface of the graft. New vessels were noted. This neoplastic tissue was covered by one layer of synovial cells. We concluded that the semitendinosus tendon can offer stabilization and the base for development of new vascularized connective tissue and it can guarantee the long term results of the anterior cruciate ligament reconstruction.
