Of the surgical conditions complicating pregnancy, diseases of the biliary tract are the most common after acute appendicitis. Despite this, there is a relative paucity of objective information concerning the incidence, aetiology and surgical management of biliary tract disease during pregnancy and its effect, if any, on the fetus. During the last 2 years 330 pregnant women were prospectively investigated at the beginning of the pregnancy and at the end of every trimester, according to the protocol. The main tests at every period of the study were, real time ultrasonography of liver, biliary tract, pancreas and kidneys, as well as biochemical assays as bilirubin, transaminase, alkaline phosphatase, LDH, serum amylase, cholesterol, triglyceridia. Two women had asymptomatic cholelithiasis at the beginning of the pregnancy (0,6%). Six women developed cholelithiasis after the second trimester of the pregnancy (1,8%). One of these six patients developed cholelithiasis during their first pregnancy, three during the second pregnancy and two in the third pregnancy. A second pregnancy is more dangerous than a first one for gallstone formation (P<0,025), but there is no difference between second and third pregnancy. Only in one patient occurred some light attacks in the last trimester of pregnancy. In all women an increased volume of gallbladder as well as an incomplete emptying of it leaving a large residual volume, were found after the second trimester of the pregnancy. From the biochemical assays progressively increased levels of cholesterol, triglyceridia and alkaline phosphatase, were found in all pregnant women. In the six cases who developed cholelithiasis during pregnancy statistical significant increased levels of cholesterol and triglyceridia were observed at the end of the third trimester of the pregnancy versus non calculous pregnant women (P<0,05). An of small degree of asympromatic hydronephrosis was developed in 41,5% of the pregnant women after the second trimester of the pregnancy. In conclusion changes in cholesterol and triglyceridia levels during pregnancy as well as incomplete emptying of the gallbladder in late pregnancy, leaving a large residual volume are the possible aetiologic factors, which suggest that pregnancy increases the risk of gallstone formation, which were incidentally noted in 1,8% of pregnant women of the northwest Greece.
