Κατάγματα της περιοχής του αυχένος του μηριαίου οστού στα παιδιά

Fractures of the neck of the femur are rare among children and are usually produced by severe violence. The recommended treatment of a hip fracture in the elderly should be avoided in children below 14 years of age. The indicated treatment depends on the type of the fracture, the angle of the fracture line, the degree of displacement and the age of the patient. In comparison with other fractures there are serious and frequent complications that may lead to permanent loss of the functional ability of the hip. This study is based on 25 patients treated in the Department of Paediatric Orthopaedics of the Aristotelian University of Thessaloniki from 1982 to 1990. Our purpose was to determine the indicated treatment as well as prognosis and complications of such fractures in addition to a wide review of the literature on this subject. We concluded that conservative treatment is recommended for the undisplaced fracture. In our opinion all displaced hip fractures, with the exception of traumatic displacement of the upper femoral epiphysis, may be also treated conservatively, provided that an urgent satisfactory reduction is achieved and maintained by traction after injury. The frequency of complications in our patients was quite low compared with that of international publications. Despite the appearance of ischemic necrosis or premature epiphyseal fusion in six patients half of them showed spontaneons improvement, while secondary surgical procedures in the rest provided an almost-normal function of the affected hip.
