Βιολογική εξέλιξη των επιφανειακών όγκων της ουροδόχου κύστεως
Abstract
dc:descriptionNinety four out of 101 patients, who have been treated, during the period 1976-1983, in Urological Clinic of Theagenion Medical Institute with primary, non invasive, transitional tumor of the urinary bladder, have been studied thoroughly. We tried to determine those biological factors of the tumor, who have, seriously influenced the evolution of the patients. The mean age of our patients was 59,3 years and the men were fivefold from the women. More than half of these patients had a tumor with low grade cell differentiation. The conclusions of this study, are the following: 1) Blood group Ο(IV) rate in our patients has been found bigger (56,4%) than the mean rate in Greeks (40%). We suggest that it is possible these patients to possess a mechanism protecting the bladder wall of tumor invasion. 2) At their first appearance, these superficial tumors were solitary in 67% and were localised around the ureteral orifices and two side walls of the bladder, in 87% of the cases. A preference of localisation on right bladder wall, which has been found, may be casual. 3) The recurrences of these tumors on the bladder vault, were threefold than the localization of the tumors on this area at their first appearance, possibly because of medical dispersion. 4) The recurrence rate in three and five years of follow-up was 60% and 71%, respectively. 5) The tumor size is the most important factor which influences the recurrence rate. In three years 83% of the "big" tumors (>2 cm) and 23% of the small tumors (<2 cm), have recurrenced. 6) The number of the tumors is the second important factor, which influences the recurrence rate. In five years, 57% of the solitary and 95% of the multifocal tumors, have recurrenced. 7) The grade of the tumor is the third important factor, which influences the recurrence rate. The intravesical chemotherapy had a similar influence. 8) Thiotepa and Adriamycin showed a similar efficacy, comparatively. They diminished (18-19%) the number of patients who recurrenced in three years and elongated (5-6 weeks) the Disease Free Interval (D.F.I.). There are indications that they influenced favorably the survival of the patients, too. 9) The disease has been made invasive in 18% of our patients, if we calculate the patients who died from the tumor. 10) The crude survival of our patients was 91.5% in three years and 86% in five years.
