Abstract
dc:descriptionAim of the study was the assessment of rubber band ligation (R.B.L.) for the treatment of haemorrhoids during the last 9,5 years (2/1978-10/1987). One hundred and ninety-five patients with haemorrhoids of all degrees were treated in our department during the above period. Of these, 111 had conservative management with application of R.B.L. In this study, however, the results of 94 patients which completed at least one year follow-up period are analysed. Fifty-four of our patients were male (Mean age: 44,75 years) and 40 female (Mean age: 44,82). Forty-five patients had first degree haemorrhoids, 39 second degree and 10 third degree. The application of R.B.L. was carried out in the out-patient proctologic clinic without general anaesthesia or sedation after liquid diet and mechanical preparation of the bowel (extract of senna leaves aperient) during the previous day. Seventy-two patients (76,59%) had no postligation symptoms or had slight discomfort-pain of less than 2 days duration, 19 patients (20,21%) had discomfort-pain for more than 3 days and 3 patients (3,19%) had severe symptoms needed analgesia or hospital admition. The postligation time off work ranged from 0-10 days but in 76% of the patients this time ranged from 0 to 2 days. The follow-up period ranged from 13-72 months (Mean: 31 months). Complications following R.B.L. were seen in 11,7% of our patients but only 2 patients (2,12%) had an urgent admition in the hospital due to complication. The results of R.B.L. technique were excellent or good in 74 patients (78,72%), fair in 16 patients (17,02%) and poor in 4 patients (4,25%). Analysis of the above results showed that in the first degree haemorrhoids almost all the patients (97,76%) had excellent or good results, while 69,22% of the patients with second degree haemorrhoids and 30% with third degree had excellent or good results. More detailed analysis of the results concerning the most worrying symptom, showed that R.B.L. had excellent or good results for: a) bleeding in 84% of patients, b) pain in 77,27%, c) prolapse in 56,25% and d) pruritus in 100%. Conclusions: 1. R.B.L. was simple in application, well tolerated from the patients, without major complications and with good results in the majority of the patients with first and second degree haemorrhoids. 2. Our results showed that there is not a clear indication for R.B.L. in third degree haemorrhoids, and 3. R.B.L. proved to be very effective for patients having bleeding as the most worrying symptom of haemorrhoids and less effective in those with prolapse.
