Abstract
dc:descriptionThe fractures of the lateral condyle of the humerus in children remain a topic controversy for the orthopaedic surgeons. A review of the relevant literature reveals that there are still unanswered questions related to: a. Mechanism of injury b. Classification of the form of the traumatic lesion. c. Indications for surgical or conservative treatment. It is our opinion that the inability to clarify the questions mentioned above is a result of the literature which is limited and refers usually to small number of cases. Our study includes 126 cases treated in our clinic and followed up from 1 to 18 years usually by the same surgeons. From the study of this series we came to following conclusions: 1. Even though type I fractures (Watsom-Jones classification) are considered stable and it is that they are treated conservatively, in two of our cases the fracture was displaced during treatment (Fig. 20,22). Therefore, we suggest that radiological supervision is necessary in these fractures. 2. Type II fractures always need surgical treatment in order to avoid complications like monunion, malunion, varus or valgus deformities e.t.c. 3. Mercer Rang's suggestion for radiological control of fracture stability with application of varus stress on the extended elbow under general anaesthesia, has only theoretical value. This is because of the risk of damaging the cartilagenous part of the epiphysis which offers stabilisation to the fracture. Although there are different approaches related to the various types of fractures, creation mechanism and indications for treatment we can state that: The type I fracture should be treated conservatively under radiological supervision to avoid complication while type II and III fractures require surgery. In our attempt to classify our series of 126 cases, 16 of them did not fall in any of the previous types. This was mainly when the fracture of the lateral condyle was accompapied by additional traumatic lesions of the elbow. Therefore we created a group IV for these fractures which gave us valuable informations for the mechanism of injury.
