Ουροδυναμική αξιολόγηση της εγχειρητικής διόρθωσης της γυναικείας ακράτειας ούρων με τις τεχνικές Stamey και Burch

dc:descriptionThe result of surgical repair of female urinary incontinence by the Burch or Stamey techniques was evaluated by using urodynamic measurements. A total of 51 cases were studied prospectively and divided into two groups; the first group included 27 patients subjected to Burch colposuspension whereas the second group included 24 patients subjected to Stamey colposuspension. The selection of either of the two techniques, for all patients, was random. All patients were preoperatively subjected to detailed clinical and urodynamic assesment and the final preoperative diagnosis was that 41 patients suffered from genuine stress incontinence whereas the remaining 10 patients had urgency as well. The postoperative urodynamic evaluation was conducted after a period of at least six months following the colposuspension operation and it was performed by using the same methodology as in the preoperative cases. The percentage of sucessfull result of the operation was 89% for the group of patients subjected to Burch technique and 83% for the group of Stamey technique. The number of cases with the postoperative complications of urgency and/unstable detrusor was minimized due to the correct preoperative selection of patients. The complications in general, were few and insignificant; it was verified though that both techniques consistently result in a postoperative significant residual volume of urine in the bladder. The analysis of data showed that both techniques result in a significant statistical decrease of the maximum flow rate and increase of the functional urethral length and of the maximum closure pressure. More specifically, it was found that the Burch colposuspension results in a greater change of the maximum flow rate and of the functional urethral length, compared to Stamey technique. Further statistical analysis of data showed that the outcome of the operation is influenced by the presence of preoperative urgency, urinary infections and by previous gynecological operations. With regard to urodynamic parameters, the outcome of the operation was found to depend on the preoperative residual volume, the postoperative first sensation, the pre-and postoperative capacity of bladder, the pre-and postoperative functional length and postoperative maximum flow rate. Finally, taking into consideration that the colposuspension Burch is technically more difficult, has a slightly higher percentage of success and is slightly more prone to complications and longer hospitalization, compared to Stamey technique, it can be concluded that both techniques are equivalent.
