A prospective study was conducted to investigate uterine activity in the active phase of labour with the use of a Gaeltec intrauterine transducer tip pressure catheter. Twenty five primigravidas and fourty three multigravidas were studied after the exclusion of 9 primigravidas (20%) and 5 multigravgidas (7%) from a random sample of 82 patients admitted in labour. Uterine activity was calculated in UAI, with an FM6 sonicaid fetal monitor and in MU from the contractions on the cardiotocogram. Total uterine activity and the contraction characteristics were assesses also. All labours lasted less than 4 hours (170±80 min for primigravidas and 99±82 min for multigravidas), since progression in labour of ≥1 cm/1 hr was considered normal. Patients that progressed slower than this were excluded. The median values for primigravid labours were: 1129 kPas/15 min, 175 MU with total uterine activity of 10.273 kPas. Multigravidas progressed faster with less uterine activity. The median values for multigravidas were 1059 kPas/15 min, 161 MU, with total uterine activity of 4.953 kPas. Uterine activity increased by 70% in primigravidas and 60% in multigravidas in the course of labour. The lowest value compatible with normal progress was 700 kPas/15 min for primigravidas and 400 kPas/15 min for multigravidas. The use of oxytocin to achieve uterine activity of 1000-1500 kPas/15 min is considered to be safe, since none of the fetuses is this study showed any evidence of fetal distress within this range of uterine activity. UAI correlates best with the rate of cervical dilatation while MU correlates less. Contraction characteristics (intensity - frequency - duration) were not significantly different in both groups and did not change significantly during the course of labour. Contraction frequency was 6-7 contractions per 15 min in both groups but did not correlate with the speed of dilatation. Almost 80% of the contractions lasted 60-120 sec with and showed intersity of 25-75 mm Hg. It is concluded that in our population succesful delivery is achieved with low values of uterine activity without significant functional dystocia.
