This classification incorporates the traditional radiological phases of bone turnover or remodelling: that is, the osteolytic phase, the mixed phase, and the osteoblastic phase. Normal or diminished bone scan activity in the osteoblastic phase indicates diminished bone turnover which we distinguish as the osteosclerotic phase. Modelling states or bone deformities of expansion or contraction are recorded as a function of bone drift at the periosteal and endosteal envelopes. A case of Pagetic bone contraction is documented. Out of a series of 112 symptomatic patients, 527 Pagetic bones were classified. Eighty-eight patients were treated medically; twenty showed progression to a more bone forming phase without a change of the modelling state, and twelve patients showed regression to an increasing bone resorptive phase. No treated patient returned to normal bone architecture. Unusual radiological manifestations of Paget's disease are incorporated into the dynamic classification. Of the drugs for the treatment of the disease available diphosphonates (EHDP) seem to be the more appropriate for the convenience of use, the absence of refractory effects and the alimination of complications, although calcitonin and mithramycin, within their limitations, have their indications in dealing with this peculiar but crippling disease.","abstract_html":"We propose a new dynamic radiological classification of Paget&#x27;s disease. Εάν πράγματι η νόσος οφείλεται σε ιό τότε ο ιός ή προϊόντα του χρησιμεύουν σαν ερεθίσματα για την ενεργοποίηση και ανώμαλη εκτροπή των μονάδων οστικής αναδόμησης (bone remodeling units). Η νέα δυναμική ακτινολογική κατάταξη της νόσου με τη χρήση των εννοιών του οστικού μετασχηματισμού και της οστικής αναδόμησης σε συνδυασμό με τον σπινθηρογραφικό έλεγχο των οστών, μας δίνει τη δυνατότητα του διαφορισμού της νόσου σε 4 συγκεκριμένες φάσεις εξέλιξής τους καθώς και τις βάσεις για να εξηγηθούν ορισμένες ασυνήθεις εκδηλώσεις της νόσου. Η επιτυχής φαρμακευτική θεραπεία συμβάλλει στην επιτάχυνση των φάσεων οστικής αναδόμησης προς περισσότερο οστεοπαραγωγική φάση και έτσι ελαττώνονται οι πιθανότητες μεγάλης παραμόρφωσης των οστών.
