Abstract
dc:descriptionThe purpose of this research was to investigate the profile of the subgingival bacterial flora which is related to the periodontal lesions in adult patients suffering from destructive periodontal disease. Furthermore, we examined the observed changes of the subgingival bacterial flora during theactive and inactive periods of the disease. Sixty four subgingival plaque samples were taken from eight patients aging 34 to 47 years. There were made two sequential clinical measurements.The lesions were categorized according to their attachment loss, as severe, exhibiting attachment loss >6 mm, moderate between 4 to 6 mm and those with an attachment loss ranging from 0 to <4 mm. In addition, in order to examine the observed alterations of the subgingival bacterial flora during periods of exacerbation and remission, the lesions were also separated into active, those who showed further loss of attachment and inactive which did not exibited further loss of attachment, during the examination period. Samples of subgingival bacterial flora were collected with three paper points No 30. They were placed in reduced transport fluid (RTF), serially diluted in 10-fold steps. This suspension was incubated anaerobically for 5-15 days at 37°C. The identification of the bacteria was made according to the Virginia Polytechnic Institute suggested methods with the simultaneous use of Minitek and API System. The dominant bacteria in the severe periodontal lesions were B. gingivalis, B. intermedius (p<0.001), C. sputigena, F. nucleatum (p<0.01) and E.corrodens (p<0.05). In the moderate periodontal lesions appeared a predominance of anaerobic Gram+ cocci but there were isolated in high levels as well B. gingivalis, B. intermedius, C. sputigena, F. nucleatum and E. corrodens. Lastly, in the periodontal lesions with a loss of attachment <4 mm, H. segnis, anaerobic Gram+ cocci and Actinomyces sps, A. odontolyticus, A.israelii, A. naeslundii and A. viscosus dominated. As far as the active periodontal lesions is concerned, there was a predominance of B. gingivalis, B. intermedius and F. nucleatum (p<0.001), C. sputigena (p<0.01) and Camp, sputorum (p<0.05), whereas in the inactive periodontal lesions anaerobic Gram+ cocci and Actinomyces sps dominated. The results lead to the following conclusions: 1) The isolation, in the severe periodontal lesions (LA>6mm), of the periopathogenic bacteria such as B. gingivalis, B. intermedius, C. sputigena, F. nucleatum and E. corrodens in high levels and frequency, is indicative of the relation that exists between those bacteria and the disease. 2) The moderate periodontal lesions flora is similar to that of the severe periodontal lesions and probably constitutes an early stage of them. 3) The isolation in the periodontal lesions with attachment loss of <4 mm, of H. segnis, anaerobic Gramm+ cocci and Actinomyces sps. and the simultaneous decrease of the periopathogenic bacteria levels, suggests that these lesions are related to a flora compatible to the healthy periodontium. 4) From the active periodontal lesions there were isolated B. gingivalis, B. intermedius, C. sputigena, F. nucleatum and Camp, sputorum in high levels and frequency, suggesting that the isolated flora, which these microorganisms dominate, is related with the exarcebation stage of the disease. 5) In the active periodontal lessions there were isolated anaerobic Gram+ cocci and Actinomyces sps. in high levels indicating that such a subgingival bacterial flora is related to the remission stages of the disease.
