Abstract
dc:descriptionSubgingival plaque samples were taken from 142 diseased subgingival sites exhibiting attachment loss >6mm, between 4mm and 6mm and <4mm in 10 young adults ranging in age from 25 to 35 years with rapidly progressive periodontitis. The samples were placed in 0.9 ml of prereducedanaerobically sterilized R.T.F., dispersed, diluted and plated on selective (T.S.A.B.) and non-selective (E.T.S.A.) media. After 7 days of anaerobic incubation, isolates were picked, subcultured, characterized and identified to the species level. The purpose of the: present investigation was to determine the predominant species in each group of periodontal lesions in relation to attachmentlevel and to reveal the differences and variability within the three clinically different periodontal lesions. As it is well known that the destruction of periodontal tissues progresses with periods of exacerbation and remission, an attempt was made to relate microbial species to these periods of disease activity. Active diseased sites comprised the sites showing increased attachment lossbetween two consecutive clinical measurments and inactive the sites showing no changes in attachment level during the same time. The results supported the following conclusions: 1. The consistent isolation of the bacterial species B. gingivalis, F. nucleatum, Fusiform bacteroides, W. recta, C. sputorum, S. sputigena, P. micros, E. alactolyticum, A. actinomycetemcomitans, E. corrodens and A israelii from the subgingival plaque samples of periodontal lesions exhibitingattachment loss > 6mm confirms further the earlier aspects that these bacterial species may be the etiologic agents of the periodontal destruction, since they have been shown to have pathogenic potential and produce virulence factors. These bacterial species either along or in combinations seems very probable to relate to the periodontal destruction and to rapidly progressive periodontitis. 2. The qualitative and quantitative differences detected in the bacterial flora between the two groups of clinically distinct periodontal lesions, severe and intermediate, revealed a variability within the microflora according to the clinical features of the periodontal lesions. 3. The bacterial flora of lesions showing attachment loss < 4mm was characterized by the consistent isolation of A. viscosus, A. naeslundii. A. propionica, C. gingivalis, H. segnis, H. aphrophi!us,H. paraphrophilus andP. acnes and the decrease of pathogenic microbiota respectively. These bacterial species appear to confer protection or they are at least compatible to a stable periodontal status. 4. B. gingivalis, F. nucleatum, Fusiform bacteroides, W. recta and C. sputorum were detected more frequently and in higher levels in active sites showing a strong correlation with active periodontal lesions. 5. A naeslundii, A. viscosus, H. segnis, L. buccalis and V. parvula were found more often and in higher levels in inactive sites showing a strong correlation with inactive periodontal lesions. These species appear to lessen the chance of a site being active and possibly confer protection. The data from the above investigation suggest that certain microbial complexes consisting of B. gingivalis, F. nucleatum, Fusiform bacteroides, W. recta and C. sputorum are associated with predominantly active sites and severe periodontal lesions. The majority of periodontal lesions which showedevidence of activity within the short space of one week indicates that the type of periodontitis we have studied is «rapidly progressive» according to the definition.
