Μελέτη της πιθανής μεταβολής της θέσης ανάπαυσης της κάτω γνάθου μετά την εξαγωγή των δοντιών και την τοποθέτηση ολικών οδοντοστοιχιών

One of the most important aspects of complete denture construction is the determination of the «correct» vertical dimension of occlusion. Although many methods have been suggested over the years for that purpose the most clinically reliable seems to be that which relates the occlusal vertical dimension to the rest position of the mandible. The primary purpose of this investigation was to determine any changes which may occur in the rest vertical dimension in the same adult patients after the removal of the remaining teeth and the insertion of complete dentures. The study continued through the 12 first months following the prosthetic treatment. The secondary aim was to study any changes in the Gonial Angle of the mandible (remodeling) through the same period.A total of 29 patients (13 men and 16 women) with at least of 10 teeth acting as sufficient occlusal stops and needing extraction were used in this study. Serial lateral cephalometric roentgenograms were made for each of the 29 patients with the mandible in the rest position at three occasions a) pre-extraction b) 7-10 days after the initial insertion of the dentures and c) 12 months later. The rest position was determined by asking the patient to swallow and relax. Two linear and three angular measurements were made for each roentgenogram and compared. The conclusions that can be drawn from this study have as follows: A. The rest vertical dimension was increased to a significant level after the insertion of the dentures. B. This increase was followed by a significant gradual reduction of the rest vertical dimension in the 12 month period of wearing the dentures.C.By the end of the 12 month period the rest vertical dimension was almost at the same level that existed before the extraction of the remaining teeth. D. The gonial angle was significantly increased after the extraction of the remaining teeth and the 12 month wearing of complete dentures.
