Abstract
dc:descriptionAfter a short review of the technique of Chronic Intermitent Hemodialysis the two main methods of vascular access are discussed. These two methods are: a) The "shunt" or external arterio-venous (Αν) communication and b) The "Fistula" or internal A-V communication. Following this, the basic principles that govern the blood flow in the vessels are mentioned and the hemodynamic behavior of the A-V communication is analyzed. Closing the general introduction we display the general principles of the "electromagnetic flowmetry" since this method was used for the study of the blood flow in our experimental model. Consequently we describe the design of the experiment, the method the A-V fistula is created and the external shunt is connected. This "mixed" vascular access was studied hemodynamically by recording the pressures and the flow rates at different points of the experimental prototype. Our results show that the hemodynamic conditions existing at the various points of the "mixed" (fistula-shunt) vascular access provide satisfactory blood flow and patency through both parts of the "mixed access". It is underlined in the discussion that the blood flow through the shunt and fistula follows and is governed by the existing hemodynamic conditions. Also the transfer of the experimental prototype to human subjects would provide a new way of vascular access with permits immediate hemodialysis, while, simultaneously, it provides the requirements for chronic hemodialysis avoiding a second operation. On the other hand it is not necessary to sacrifice the couple of vessels used. The basic requirements for the normal function and patency of this vascular access are: a) the presence of a normal central artery which provides an adequate blood pressure and is capable of increasing the blood flow, b) The meticulous technique of a wide A-V communication and connection of external shunt. In conclusion the method of the "mixed" vascular access beyond the theoretical and experimental interest is of important clinical application in patients who need immediate hemodialysis and who at the same time have the need for further chronic intermittent hemodialysis. This is because the time required for maturing of the veins of an A-V fistula is by passed and also because two important vessels are saved for probable use in the future.
