ΜΕΛΕΤΗ ΠΑΡΑΜΕΤΡΩΝ ΕΓΚΕΦΑΛΟΝΩΤΙΑΙΟΥ ΥΓΡΟΥ ΣΕ ΘΕΡΑΠΕΙΑ ΠΕΙΡΑΜΑΤΙΚΗΣ ΜΗΝΙΓΓΙΤΙΔΑΣ ΑΠΟ HAEMOPHILUS INFLUENZAE TYPE B
Abstract
dc:descriptionIN THIS STUDY THE DIFFERENT ASPECTS OF PATHOPHYSIOLOGY AND VARIOUS TREATMENT MODALITIES IN EXPERIMENTAL HAEMOPHILUS INFLUENZAE TYPE B (HIB) MENINGITIS WERE ASSESSED. IN THE FIRST PART THE ROLE OF HIB CAPSULE AND LIPOPOLYSACCHARIDE IN THE INDUCTION OF MENNIGEAL INFLAMMATION WERE EVALUATED. A MINIMAL INFLAMMATORYRESPONSE WAS FOUND FOLLOWING INTRACISTERNAL INOCULATION OF PURIFIED HIB CAPSULAR POLYSACCHARIDE (POLYRIBOSYLRIBITOL; PRP). IN CONTRAST TO PRP, THE INOCULATION OF PURIFIED HIB LIPOPOLYSACCHARIDE WAS FOLLOWED BY SIGNIFICANT CSF PLEOCYTOSIS AND INCREASED CSF PROTEIN CONCENTRATIONS, IN A DOSE-DEPENDENT FASHION (R=0,8147 AND R=0,6069, RESPECTIVELY). SIMILAR CONCENTRATION OF FREE LIPOPOLYSACCHARIDE IN CSF IS OBSERVED AFTER THE USE OF BACTERIOLYTIC AND ANTIBIOTICS IN THE TREATMENT OF BACTERIAL MENINGITIS. IN ADDITION THE VARIOUS ENDOGENOUS AND EXOGENOUS FACTORS, THAT MODIFY THE INFLAMMATORY ACTIVITY OF LIPOPOLYSACCHARIDEWERE ASSESSED. THE ADDITION OF POLYMYXIN B INDUCED SIGNIFICANT REDUCTION OF CSF WBC AND PROTEIN LEVELS (P < 0.05). PREINCUBATION OF LIPOPOLYSACCHARIDE WITHA MONOCLONAL ANTIBODY AGAINST HIB LPS CORE OLIGOSACCHARIDE DID NOT DECREASE CSF INFLAMMATION, WHEREAS DEACYLATION OF THE LIPID A MOIETY OF LPS WITH NEUTROPHIL ACYL-OXYACYL HYDROLASE PRODUCED A SUBSTANTIAL REDUCTION IN MENINGEAL INFLAMMATION. IN THE SECOND PART OF THE STUDY THE EFFECT OF TREATMENT IN MENINGITIS WAS EVALUATED. THE BRAIN WATER CONTENT AFTER TREATMENT WITH DEXAMETHASONE ALONE, DEXAMETHASONE IN COMBINATION WITH CEFTRIAXONE OR CEFTRIAXONE ALONE WAS 404 +-12, 406+-12 AND 411+-14, RESPECTIVELY, VERSUS 419+-10 GM H2O/100 GM DRY WEIGHT IN UNTREATED INFECTED RABBITS (P < 0.05, P < 0.05 AND P < 0.1, RESPECTIVELY, FOR THE THREE TREATMENT MODALITIES, IN COMPARISON WITH THE UNTREATED (CONTROL) GROUP I). FINALLY, THE ADMINISTRATION OF A SINGLE DOSE OF MANNITOL 25% INDUCED A SIGNIFICANT, BUT TEMPORARY REDUCTION OF INTRACRANIAL PRESSURE, FROM 4,78+-77 TO 2,61+-1, 82 MMHG (MEAN REDUCTION 42%, P < 0.05) AND A SIGNIFICANT DECREASE OF CSF LACTATE CONCENTRATION (FROM 71,8+-17,0 TO 59,2+-19,2 MG/DL; P <0,05).
