Je 50 MSSA und 50 MRSA aus dem ambulanten und nosokomialen Bereich wurden mittels der Pulsed-field Gelelektrophorese (PFGE) typisiert. Dabei konnten bei den ambulanten MSSA 26 Genotypen unterschieden werden. Bei den nosokomialen MSSA ergaben sich 24 Genotypen, bei den ambulanten MRSA 13 Genotypen und bei den nosokomialen MRSA 11 Genotypen. Damit wiesen die MRSA insgesamt eine niedrigere genetische Diversität als die MSSA auf. Die nosokomialen MRSA waren außerdem genetisch weniger divers als die ambulanten MRSA. Zwei MRSA-Genotypen A und B dominierten sowohl im ambulanten als auch im nosokomialen Bereich. Typ A zeigte sich als dem sogenannten „epidemischen Rhein-Hessen-Stamm“ zugehörig. Typ B ist dem Fragmentmuster nach in die weitere Verwandtschaft eines Epidemiestammes aus Frankreich einzuordnen. Die Untersuchung auf eine mögliche S. aureus-Transmission im Universitätsklinikum Freiburg ergab bei der Mehrzahl der nosokomialen Isolate der vorherrschenden MRSA- und MSSA-Genotypen keine räumlich-zeitliche Assoziation der stationären Aufenthalte der betroffenen Patienten. After primary cultural identification 207 methicillin-susceptible S. aureus (MSSA) and 104 methicillin-resistant S. aureus (MRSA) were analysed with the mecA femB duplex-PCR. The primary cultural identification showed to be inferior to the PCR considering the identification of the specie and of the oxacillin-resistance. 50 MSSA and 50 MRSA each of the community-based isolates and of the hospital-based isolates were typed with the pulsed-field gel electrophoresis (PFGE). The community-based MSSA could be grouped into 26 different genotypes. The hospital-based MSSA showed 24 genotypes, the community-based MRSA 13 genotypes and the hospital-based MRSA 11 genotypes. The MRSA had therefore less genetic diversity than the MSSA. The hospital-based MRSA additionally showed less diversity than the community-based MRSA. Two MRSA-genotypes A and B were predominant among the community-based and among the hospital-based isolates. Type A belonged to the “epidemic strain of Rhein-Hessen”. Type B showed a fragment pattern which could be related to an epidemic strain from France. The investigation of a possible transmission of S. aureus among the patients with the predominant MRSA- and MSSA-genotypes within the University Hospital of Freiburg showed for most of the isolates that there was no correlation of the hospital stays of the infected patients. 70 of the patients with community-based MRSA had an in-patient treatment during the last year before the isolation of S. aureus. Among the predominant types A and B this rate was even higher than among the other non-epidemic genotypes.
