We reviewed the case records of 227 consecutively diagnosed South African patients with XDR-TB between 2002 and 2008, and analysed the records of another 115 patients, retrospectively, for adverse drug reactions (ADRs). It was found that a significant proportion of XDR-TB patients are HIV unrelated, and prognosis, regardless of HIV status, poor. Nevertheless, survival in HIV infected patients is better than previously reported, and treatment with HAART improves outcomes. The frequency of ADR’s with XDR-TB treatment regimens is high, often severe, and negatively impacts on culture conversion outcomes.
