A importância do seguimento farmacoterapêutico na saúde : uma revisão da literatura.

dc:description.abstractThe pharmaceutical care program favor to achieve positive results in pharmacotherapy. Thus, the pharmaceutical acts promotes the rational use and correct use of drugs, successful treatment, patient compliance and minimize prescriptions errors. The pharmaco therapy follow - up program enable pharmacist to apply his knowledge about health problems and drugs. It works through research, prevention and resolutions of drug related problems. This study aimed to produce a review article on P harmacotherapy follow - up and encourage the practice of pharmaceutical care. This is a bibliographic study that was developed using several databases. The most used methods are SOAP (subjective, objective, assessment, plan), TOM (Therapeutic Outcomes Monitoring), Pharmacotherapy Wo rkup and method Dáder. The four pharmacotherapy follow up programs were detailed as the methodologies and applications. Many a rticles were found that proves positive clinical results and resolution of drug related problems of patients that had different pa thologies, motivated by the pharmaceutical care program. All pharmacotherapy follow up program proved effective in the practice of pharmaceutical care. It weren't find factors that show difference in performance between methods. On the other side, each one can be apply in different occasion because they have different methodology . All methods when compared presented advantages and disadvantages. T he choice depends on the training and professional practice.
