Ações programáticas de controle do diabetes mellitus desenvolvidas por equipes de saúde da família na Bahia

Diabetes mellitus is a disease of great prevalence in Brazil. Access to primary health care of quality makes it possible to offer continuous actions of health promotion and prevention against complications to those who suffer from this disease. The objective of this study is to characterize programmatic actions of Diabetes Mellitus control carried out by Family Health teams that joined the National Program for Improving Access and Quality of Primary Care (PMAQ). From the health teams of Bahia 1464 they answered the PMAQ questionnaire. Twenty-three variables were extracted from the public database of the first cycle of the Program for the year 2012. The results were described in relative and absolute frequencies. Regarding the scheduling and registration of people with diabetes, 53.1% of the health teams in Bahia were scheduled on any day of the week, at any time of the day, 96.7% of the teams used a registration and follow-up form and the time waiting for the first consultation had an average of 5.2 days between the teams. The waiting time for specialists cardiologist, nephrologist, ophthalmologist and endocrinologist had the median of 30 days. Regarding follow-up examinations and complications screening, 52.9% of the teams studied did not have patients with a higher risk or severity, 93.1% of the teams studied required glycosylated hemoglobin, 34.6% of the teams had references and defined flows for the glucose test, 59.6% of the teams underwent diabetic foot examination and 35.7% of the teams underwent eye examination. Regarding educational and health promotion actions, 92.2% of the teams in Bahia carried out these actions. In order to reach the diabetic, 88.3% of the teams performed an active search in the territory. Of the teams studied, 69.6% had communication channels with the user. In spite of advances in the access of people with diabetes to actions carried out by the Family Health teams, weaknesses were observed in the coordination of care. It is concluded that the family health teams that joined the first cycle of the PMAQ in Bahia need to improve the quality of care for the diabetic patient, especially regarding the identification and monitoring of lesions in target organs, which would require a greater investment in the coordination of the care of this condition by primary health care.
