Conception and Implementation of a Pharmacogenomic Plausibility Check: Integrating Molecular Data to Optimize Established Drug Therapy Safety Workflows

dc:description.abstractIn recent years, there has been a notable expansion in the accessibility of high-quality pharmacogenomic knowledge and research exploring its advantages. The increasing availability of freely accessible information on drug-gene interactions enables the use of such data for a plausibility check during the prescribing process, thereby enhancing the safety of drug therapy. By conducting a pre-emptive test of the genotype of relevant enzymes, the result can be incorporated into the patient profile in advance, in a manner analogous to the inclusion of other patient characteristics. In this study, the existing ID PHARMA CHECK® software from ID Berlin, which is used in numerous hospital information systems to enhance the precision of drug therapy, is enhanced with a molecular component. The new data warehouse, GraphSAW2, is used to collate the pharmacogenomic knowledge. A pharmacogenomic plausibility check will be conceptualised and developed using the newly collected knowledge. New methods for the extraction, storage and processing of pharmacogenomic knowledge, along with an infrastructure for the storage of genomic patient data, will be developed. The results will be presented to healthcare professionals in a consistent format, modelled on other existing drug plausibility warnings. The introduction of a new severity level for each pharmacogenomic alert allows for optimised prioritisation, effectively avoiding the problem of over-validation. The implementation of this new approach to evaluating drug therapy safety at the molecular level in hospitals represents a significant advance in the era of precision medicine.
