Infektionsprävention in der außerklinischen Intensivpflege – Ergebnisse eines Scoping Reviews und einer Prävalenzstudie zu Hygienestrukturen sowie dem Vorkommen von Infektionen, Antibiotikagaben und Multiresistenten Erregern

dc:description.abstractIn Germany, more and more patients in need of home mechanical ventilation (HMV). According to current knowledge, these patients are characterized by high device application rates and evidence of multi-resistant pathogens (MDRO). So far, there are only a few, regionally limited studies on the occurrence of infections and MDROs in HMV. This area of research should be examined more closely as part of a prevalence study. This dissertation describes the situation and the challenges in AIP, especially with regard to infections and MDRO. Within the framework of a systematic literature research, the current findings from the international literature are presented. So far there are only a few publications on hygiene or infection prevention in the HMV. Based on the literature, however, it can be seen that MDRO are often detected in patients in HMVP - however, there are no studies that record the prevalence of infections in this care setting. Therefore, a separate prevalence study based on the HALT study (Healthcare-associated Infections in European Long Term Care Facilities) was carried out. The corresponding questionnaire was modified in terms of language and in some cases also content in order to adapt it to the special properties of the HMV. 38 ambulant care services from eight federal states took part in the online study. In total, data on 288 patients could be collected. The present prevalence study shows that the previous assumption of a high MDRO prevalence is justified and that there are different MDRO prevalences in the respective forms of care. Infections were only diagnosed in 25 patients and entered in the questionnaire. Based on these results, recommendations for action for medical staff and those responsible for facilities in the AIP are formulated.
