İnflamatuvar bağırsak hastalıklarında helicobacter pylori ve dispepsi prevalansı

dc:description.abstractCrohn's disease and Ulcerative colitis, the two major inflammatory disease of intestine, are characterized by chronic and recurrent immun activation and inflammation. The pathogenesis underlying the two disease is similar but exact cause is not known. There are many studies with different results with respect to Helicobacter pylori infection rates in that its incidence is low in both of these diseases. The aim of this study is to point out whether there is difference between Ulcerative colitis and Crohn's disease regarding the frequency of Helicobacter pylori infection. Our purpose is also to evaluate how Helicobacter pylori distibution changes with treatment. The other purpose of the study is to investigate the frequency of both upper gastrointestinal lesions and dyspeptic symptoms that are commonly seen in inflammatory bowel diseases. In this study, 138 patients with inflammatory bowel disease has been evaluated. Reports of 81 patients (55 patients with Ulcerative colitis; 24 male, 31 female and 26 patients with Crohn's disease; 1 1 male, 1 5 female) were included in this. Demographic characteristics of patients; disease type and affected site in disease, disease activity, lesions of upper gastrointestinal tract, Helicobacter pylori status and used drugs all noted. Patients are questioned about dyspeptic symptoms. Oesaphagus, stomach and duodenum all evaluated with upper gastrointestinal endoscopy. Lesions were classified by Sydney criteria. Culture, cytologic examination and urease test were performed and Helicobacter pylori existence was investigated in each of the 3 biopsies taken from the antrum of stomach. As a result of the study we found that Helicobacter pylori infection rate was higher in patients with Ulcerative colitis against Crohn's disease group. In each of the disease groups, Helicobacter pylori existence was not associated with the treatment differences between patients. Patients with Crohn's disease have more negative mucosal lesions for Helicobacter pylori and this result was not associated with symptoms. These findings support the idea that Crohn's disease somehow prevents the Helicobacter pylori's settlement in the mucosal layer of the stomach. The reasons for these two findings commonly seen in Crohn's disease and their importance seem to worth investigating. Novel researchs with this purpose would be a guide in the diagnosis and treatment of Crohn's disease.
