Effectiveness of Nursing Intervention for Delirium Prevention on Prevalence of Delirium and Cognitive Function of Patients in Intensive Care Unit

Delirium is a neuropsychiatric temporary syndrome that occurs in Intensive Care Unit (ICU) with acute onset. Patients with delirium are characterized by cognitive impairment, depressed consciousness, disrupted attention, increased or decreased mental activities, and sleep disorder. This study is aimed to determine the effect of Nursing Intervention for Delirium Prevention (NIDP) on prevalence of delirium and cognitive function in ICU patients. It was conducted with control group of 26 ICU patients and treatment group of 23 patients at the university hospital in Gyeonggi province. Twelve-week data from control group were collected between August and October 2011 and those from treat ment group were collected between March and May2012. Treat ment group received total6 time of NIDP: first session on the first day, three sessions on the second day, and two sessions on the third day of ICU admission. After last session, prevalence of delirium and cognitive function were measured using Confusion Assessment Method-Intensive Care Unit (CAM-ICU) and Mini-Mental Screening Examination-Korea(MMSE-K). Collected data were analyzed by descriptive statistics, χ2 test and independent t-test using PASW 18.0 software program. The results of this study are as follows: 1. First hypothesis is that the treatment group with NIDP has significantly lower prevalence rate of delirium than the control group with usual care.This hypothesis was supported by χ2 test(χ2=4.05, =.044). 2. Second hypothesis is that the treatment group with NIDP has significantly higher cognitive function than the control group with usual care. This hypothesis was supported by independent t-test( =2.52, =.015). In conclusion, application of NIDP to ICU patients resulted in statistically significant decrease of delirium prevalenceandimprovementofcognitivefunction in comparison to control group. Limitation of this study with small population warrants additional studies with larger population. In addition, fur ther study of the NIDP effect on prevalence of delirium and cognitive function is recommended in control of other delirium causing factors.
