Detection of Pepsin in the Sputum/Saliva for Diagnosing Atypical Manifestations of Gastroesophageal Reflux Disease

BACKGROUND/AIMS: Atypical manifestations of GERD are difficult to diagnose.We aimed to determine whether measurement of pepsin in the sputum/saliva could be useful for diagnosing atypical GERD manifestations. METHODS: Patients with clinically suspected atypical GERD symptoms provided sputum/saliva collected before bed,at the time of symptoms,and after awakening for pepsin measurement by western blot analysis. All subjects received 24-hr esophageal pH monitoring, and then 40 mg of esomeprazole was given twice a day for two weeks. RESULTS: The pepsin test was positive in 20 out of 40 patients with pepsin detected in 18 samples(45%) collected at the time of symptoms. Compared with 24-hr pH-metry data,the sensitivity and negative predictive value of the pepsin test were 89% and 95% in total patients, respectively. The pepsin test in each a typical symptom group exhibited higher sensitivity and negative predictive value, and lower specificity and positive predictive value. Compared with the PPI test results, the sensitivity, specificity, positive and negative predictive value ofthe pepsin test were 60%, 70%, 67%, and 64% in total patients, respectively. CONCLUSIONS: Pepsin measurement in the sputum/saliva collected at the time of symptoms appears to provide a sensitive, noninvasive method for diagnosing atypical GERD manifestations.
