Expression of Survivin : Possible Predictor of Local Recurrence after Curative Resection for Rectal Cancer

dc:descriptionLocal recurrence has always been a challenging problem following rectal cancer surgery, desite of the advances in surgical technique(TME) as well as adjuvant therapy(preoperative CCRT). There is a need for better prognostic markers due to the varieties of treatment response within same clinicopathologic stage. Survivin, a novel member of IAP protein, is often associated with increasing recurrence risk and poor prognosis. We investigated the expression of survivin in rectal cancer and its potential impact on local recurrence. Retrospective clinicopathological data were analysed in 23 patients of local relapse and 60 patients of no recurrence with rectal cancer, staged Ⅱ and Ⅲ. Immunohistochemical staining for p53, bcl-2, survivin and cleaved caspase-3 were performed on paraffin-embedded tissue sample of 46 rectal cancer. By clinicopathological data, risk factors for local recurrence were positive nodes (45% vs 69.6%, P<0.046) and lateral margin invasion(1.7% vs 30.4%, P<0.000). Immunohistochemistry revealed “high expression” of survivin could be a predictive parameter of local recurrence in stage III rectal cancer(29.2% vs 59.1%, P<0.043). Our results suggest that “high survivin expression” is possible risk factor of local recurrence and that survivin may be a target for cancer therapy.
