Compensation of the Cerebral Perfusion in Patients with Infarction : SPM Analysis
Abstract
dc:descriptionThe purpose of this study was to evaluate the compensatory effect on cerebral perfusion in patients with cerebral infarction. Ninety three Tc-99m ECD brain perfusion SPECTs (male/female: 58/35, mean age 58) of 93 patients with cerebral infarction were evaluated retrospectively. Any patient with bilateral cerebrovascular disease was excluded from the study. All patients were grouped into left or right lesion for SPECT , then subgrouped according to the severity of lesions for SPECT. Each subgroup was compared with normal controls in a voxel-by-voxel manner (two sample t-test, p<0.001) using SPM. Forty nine left and 44 right lesions for SPECT. And all groups for SPECT showed a significantly increased cerebral perfusion in the contralateral cerebral cortex, but not in ipsilateral cerebellum. When we compared each subgroup of SPECTs with controls, only subgroup with severe lesion in left side showed a significantly increased perfusion in ipsilateral cerebellum. In our study, perfusion SPECT showed compensatory hypermetabolism in the contralateral cerebral cortex, but compensatory hyperperfusion in cerebellum was observed only for subgroup with severe lesion in left side.
