Comparison of Urine Cytology using MonoPrep2TM system and Conventional Urine Cytology for the Diagnosis and Follow-up of Bladder Cancer

PURPOSE: Cystoscopy and urine cytology are the standard procedures for detection and surveillance of bladder cancer. Although conventional urine cytology has played a significant role as an adjunct to cystoscopy, it has several disadvantages, including a low sensitivity in well differentiated tumors, dependence on adequate numbers of cells for analysis but loss of many cells during preparation, being subject to cellular alterations from collection conditions and multilayering of cells in the slide making difficult to interpret. A new liquid-based filter monolayer preparation technique, MonoPrep2TM (MP) system, purportedly has the advantages over conventional cytology that it is simple, has high cellular yield, and provides good preservation and monolayering of cells in the slide. The MP system was used in fine-needle aspiration of breast cancer and screening of cervical cancer but has never been used in the urine cytology. Therefore, urine cytology using MP system and conventional urine cytology was compared in the diagnosis and follow-up of bladder cancer. MATERIALS AND METHODS: We compared the urine cytology using MP system with conventional urine cytology in 120 voided urine samples obtained from 108 patients, for hematuria work-up or bladder cancer follow-up, from January 2001 to December 2003. There were 84 males and 24 female (mean age 57.6 years, range 10-86). The two methods were compared with respect to cellularity, monolayering of cells, nuclear detail, staining quality, and diagnostic accuracy. RESULTS: Urine cytology using MP system was better than conventional urine cytology in terms of cellularity, monolayering of cells, nuclear detail, staining quality. The sensitivity for the detection of bladder cancer was 94.1% in urine cytology using MP system and 82.4% in conventional urine cytology, showing that urine cytology using MP system was more sensitive than conventional urine cytology, but without statistical significance (p=0.077). The specificity in urine cytology using MP system and conventional urine cytology was 81.2% and 91.3%, respectively (p<0.001). CONCLUSIONS: Urine cytology using MP system is simpler and has a better quality than conventional urine cytology. Urine cytology using MP system is more sensitive than conventional urine cytology in the detection of bladder cancer, but without statistical significance, and less specific than conventional urine cytology. Therefore, the MP system could be a useful alternative to conventional method in the urine cytology for the detection of bladder cancer, if the problems in specificity could be improved.
