Refractive Outcome in Eyes of Preterm Infants

PURPOSE: To evaluate the refractive status and factors influencing the occurrence and degree of myopia in premature infants. Patients and METHODS: Refractive errors in 86 premature infants (a gestational age <37 weeks) born between september 1994 and June 2004 were examined. They were subdivided into two groups according to treatment for threshold ROP (retinopathy of premature); group 1: children born prematurely with threshold ROP, group 2: children born prematurely without threshold ROP. Cycloplegic refraction was performed, and ultrasound biometry (A-scan) was also performed. These results were compared with the control group (group 3, full term babies). RESULTS: Between age of 4 - 6, there were significant differences in the prevalence of myopia among the three groups, and the degree of myopia was higher in premature infants born with threshold ROP than the others. There were significant differences in the prevalence of hyperopia under the age of 4, and the degree of hyperopia was higher in premature infants born without threashold ROP than the others. The prevalence and the degree of astigmatism were similar among three groups. There were no significant differences in the axial length and anterior chamber depth between group 1 and 3. The degree of myopia was related to laser or cryotherapy treated eyes, birth weight, intrauterine period, and axial length. CONCLUSION: Laser or cryotherapy treated eyes were significantly more myopic than the others. The degree of the myopia was found to be related to the birth weight, alxial length, anterior chamber depth, and intrauterine period. Premature infants should be screened regularly and long-term follow up should be recommended to prevent amblyopia, strabismus, and other ocular abnormalities.
